Your health information and your rights.

When you register as a patient, Eastbound Medical Clinic collects your personal and health information so our GPs and care team can provide you with safe, high-quality healthcare, and for directly related activities such as Medicare/DVA claiming, accreditation and practice administration.

We collect your name, date of birth, contact details, Medicare/DVA and health-fund details, and your medical history (including medications, allergies, immunisations and relevant family and social history). Information may be collected directly from you, or from other treating providers, Medicare/DVA or My Health Record as part of your care.

Your information is stored securely (electronic and locked paper records) and is only accessed by staff who need it for your care. You may request access to or correction of your record in writing to the Practice Manager. You can also deal with us anonymously or by pseudonym where practical.
