Chronic Disease Management

Compassionate nurses provide GP-supported care for patients with multiple chronic health conditions. The Care Coordination nurses will work closely with your GP and develop a plan that is personalised for you.

For ongoing conditions such as heart disease, diabetes, lung disease and arthritis, your GP can prepare a GP Management Plan and Team Care Arrangements, supported by our care-coordination nurses, so your care is proactive, structured and personalised.

• Where: on site at our Bentleigh East practice
• Care by: RACGP-accredited GPs and trained nursing staff
